    Common Carriers top

How do I become an approved carrier for TXI? 

Approved Carriers will be assigned a ìCarrier Codeî; no hauling will be permitted without a carrier code.

To obtain a ìCarrier Codeî

  1. Carriers will be required to furnish copies of ìOperating Authorityî
  2. Carriers will be required to furnish an ìInsurance Certificateî identifying TXI Operations, LP as certificate holder and showing coverageís for:
    (a)  Comprehensive General Liability $ 1,000,000
    Combined single limit for each occurrence, to include bodily injury and property damage, contractual liability and completed operations.
    (b)   Comprehensive Automobile Liability $ 1,000,000
    Combined single limit per occurrence, to include bodily injury and property damage
    (c)   Workers Compensation and Employers Liability at statutory limits

    ÖorÖ

    Accidental Death & Dismemberment/Health Insurance coverageís of:
    $ 100,000 each accidental death
    $ 100,000 each loss of limb or sight
    $ 300,000 medical insurance for each person and 60% pre-        injury income for 52 weeks per employee (maximum weekly benefit or $ 400) for loss income.
    Certificates of Insurance should include a ì30 day prior notice of cancellationî provision.
  3. Carriers will submit a completed ìForm W-9î.
  4. Carriers will complete and submitted the following information form:
